29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: a) Mandrel Area: On or about August 8, 2025, the tongue guards were approximately 1" - 4 " from the face of the abrasive wheel on multiple bench grinders, exposing employees to potential injury from wheel breakage or ejected fragments. NOTE: Abatement certification is required for this item.
