29 CFR 1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): Instance A On or about July 29, 2025, employees were exposed to amputation and struck by hazards created by the rotation of blades and the left and right movement of the blade carriage when climbing on top of the carriage to perform blade changes on the MTM line. The employer did not ensure that energy control application steps were implemented to control the equipment's electrical, hydraulic, and pneumatic energy sources prior to the servicing tasks - the employer did not: - Ensure that equipment was turned off or shut down. - Physically operate energy isolating devices to control energy sources and affix lockout device(s). - Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe. - Verify that isolation and de-energization of the machine or equipment had been accomplished. Instance B On or about July 29, 2025, employees were exposed to amputation and struck by hazards created by the rotation of blades and the left and right movement of the blade carriage when performing blade changes on the OTO line. The employer did not ensure that energy control application steps were implemented to control equipment's electrical, hydraulic, and pneumatic energy sources as well as the potential stored energy of stock material under tension prior to the clearing a jam - the employer did not: - Ensure that equipment was turned off or shut down. - Physically operate energy isolating devices to control energy sources and affix lockout device(s). - Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe. - Verify that isolation and de-energization of the machine or equipment had been accomplished. Instance E On or about July 24, 2025, employees were exposed to caught in and amputation hazards created due to the rotation of axels, sizing wheels, and guide wheels. The employer did not ensure that energy control application steps were implemented to control the equipment's electrical and hydraulic energy sources prior to making adjustments at the weld head, sizing, roughing, and turk heads on the OTO line- the employer did not: - Ensure that equipment was turned off or shut down. - Physically operate energy isolating devices to control energy sources and affix lockout device(s). - Ensure that all potentially hazardous stored or residual energy was relieved, disconnected, and rendered safe. - Verify that isolation and de-energization of the machine or equipment had been accomplished.
