29 CFR 1910.134(f)(1):The employer did not ensure that employee(s) required to use a tight-fitting facepiece respirator passed the appropriate qualitative fit test (QLFT) or quantitative fit test (QNFT): a. On or about July 28, 2025, Torch Area: employees were exposed to respiratory hazards, in that, an employees performing torch application in steel and aluminum parts using a Gerson 9350 Silicone Half Mask Face respirator was not fit tested prior to initial use of the respirator and at least annually thereafter.
