29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided. a) Boiler Room: On or about June 30, 2025, the employer exposed employees to electrical shock hazards while installing an electrical circuit, in that the employer did not provide and ensure employees used electrical protective equipment such as rubber insulating gloves nor that the circuit to be connected was deenergized before employees worked on an energized circuit.
