29 CFR 1910.146(e)(1): Before entry was authorized, the employer did not document the completion of measures required by 29 CFR 1910.146(d)(3) by preparing an entry permit. On or about 25 June 2025, Gilster-Mary Lee Corporation, located at 615 Old St. Marys Road in Perryville, Missouri, the employer allowed entry into a known permit-required confined space prior to the completion of an entry permit. An operator was required to enter a known permit-required confined space, Sugar Coat 1 Oven, to perform scheduled cleaning.
