29 CFR 1926.651 (k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person prior to the start of work, and as needed throughout the shift, for evidence of situations that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions: a) 67 Sweeny Blvd, Hampton VA: On or about June 18, 2025, employees worked inside of an excavation doing rebar placement and concrete form work while employees were setting wood in place to receive concrete. Excavated material showed signs of rolling off into the excavation. A daily inspection of the excavation was not conducted exposing employees to a hazardous condition of entrapment. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
