29 CFR 1910.147(c)(4)(ii)(D): Energy control procedures shall clearly and specifically outline specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures: On or about April 9, 2025, at a facility located at 200 North Grand Avenue in Rothschild, WI; the written "High Pressure Eductor Pump Lockout" procedure did not outline any requirements for testing or verification of the effectiveness of the inlet and outlet ball valves which were used to lockout chemical energy in the form of cooking acid (calcium bisulfite and sulfurous acid) from the High Pressure Eductor Pump in the Pulp Mill.
