Text For Citation: 01 Item/Group: 002B Hazard:
29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: a) Worksite - On or about April 14, 2025, employees were adjusting on a butterfly valve by one employee stroking a butterfly valve (Fisher 6" 8580 300# RF Butterfly Valve) using air pressure while a second employee held the butterfly valve to stabilize it with their left hand while it was being lifted up by an overhead crane (R & M, Hoist Serial # 1315428, Hoist Model # SX40410050P35FBLOS). The second employee holding and stabilizing the butterfly valve was exposed to struck by and caught-in/between hazards when the the butterfly valve was stroked with air pressure and the employee's left thumb tip was caught between the disk and the edge of the butterfly valve. The employees were not provided authorized employee lockout/tagout training by the company.
