29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l): On or about April 9, 2025, it was observed that employees performing recreational vehicle servicing and maintenance at BRV Lincoln, LLC located at 1000 and 1040 Saltillo Road, Roca, Nebraska were exposed to struck-by hazards in that the employer did not certify forklift operator training and evaluation prior to truck use.
