29 CFR 1910.147(c)(4)(ii):The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: a. On or about March 11, 2025, the specific procedures for hazardous energy control (lockout/tagout) of the RoAn Sideweld Solid 9500 bag machines did not include: [1910.147(c)(4)(ii)(B)] Specific procedural steps for shutting down, isolating, blocking, and securing bag machines to control all applicable electrical, hydraulic, pneumatic, and thermal hazardous energy sources; [1910.147(c)(4)(ii)(C)] Specific procedural steps for the placement and removal tagout devices; [1910.147(c)(4)(ii)(D)] Specific procedural steps for testing the machine or equipment to determine and verify the effectiveness of tagout devices and other energy control measures, thereby exposing employees to struck-by, caught in, and thermal hazards associated with performing servicing and/or maintenance activities.
