29 CFR 1910.333(b)(2)(iv)(B):Before circuits or equipment was worked as deenergized, a qualified person did not use test equipment to test the circuit elements and electrical parts of equipment to which employees would be exposed and did not verify that the circuit elements and equipment parts are deenergized. On or about February 08, 2025, the employer did not ensure that a qualified employee performed testing to confirm the electrical circuit elements were effectively deenergized at the NW, NE, SW and New Building substations prior to preforming maintenance and/or testing.
