29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On or about January 6, 2025, at the worksite located at 1131 Dakota Avenue, Dakota City, Nebraska, employees engaged in maintenance activities were exposed to caught-in and crushing hazards. The employer did not ensure that employees adequately lockout and/or tagout hazardous energy sources on the Newcastle Load Transfer machine, serial number: 14134. Instances included, but were not limited to: i. 120-volt electrical power and ii. Pneumatic ball valve controlling slave board apparatus (east side when facing machine).
