29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: On or about November 6, 2024, at the fabrication area of the jobsite located at 1160 West 13th St, Unit 10, Riviera Beach, Florida 33404, lockout devices were not provided for employees who performed servicing and/or maintenance of an edge banding machine (i.e. HolzHer - Model No. 1402MFE - Serial No. 1895/0-010), thus exposing employees to an amputation hazard.
