29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not conducted by the competent person prior to the start of work and as needed throughout the shift: a) 794 Angelou Avenue, Middletown, DE; On or about 10/18/2024, employee entered an excavated area without the excavation being inspected by a competent person resulting in soil collapsing onto the employee working in the excavation installing a new sewer connection.
