29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about October 21, 2024 and at times prior to that date, near 915 E 33rd St. in Edmond, Oklahoma, no protective devices were in place to protect employees working in an excavation that was more than 5 feet in depth, exposing employees to the hazard of being crushed by falling soil. The B & H construction LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1518853, citation number 1, item number 1a, and was affirmed as a final order on June 21, 2021, with respect to a workplace located at 1303 W Tecumseh Rd. Norman, Ok 73069. B&H Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1462585, citation number 1, item number 1a and was affirmed as a final order on April 23, 2020, with respect to a workplace located at 7th and Cemetery Road.
