29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation. a) At 2733 Gunter Park Dr. West - Main Area: On or about October 9, 2024, and at times prior thereto, employees were exposed to crush-by hazards when performing maintenance activities on the Dave press, in that each employee had not been trained on lockout/tagout in accordance with the standard.
