29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service: On or about 5/8/2024, and times prior thereto at this workplace employees conducted scheduled maintenance operations at wind turbine # 43-B, the hoist winch was powered by 220 volts via a flexible cord and plug-electrical connector. The flexible cord and plug-electrical connector had separation defects and were not removed from service.
