29 CFR 1910.178(p)(1):Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s) a) Warehouse floor; On or about May 8, 2024, and times prior, the employer exposed employees to struck-by hazards, in that the employer did not remove from service the "Mitsubishi 25"model 6FCC25, serial number AF 82A-52861" which had poorly functioning/faulty steering and an inoperable horn where the forklift was used by employees to move heavy carpet rolls.
