29 CFR 1910.179(j)(2)(iii):Monthly inspections of hooks, with a certification record which includes the date of inspection, the signature of the person who performed the inspection and the serial number, or other identifier, of the hook inspected, were not performed: (a) Metal Fabrication Area: on or about and at times prior to April 24, 2024, a hook used with the 15/5-ton Shaw Box S/N 38783 crane had a safety latch which was bent upwards and did not make contact with the hook. The hook was used to move steel transformer housings weighing up to 15-tons. The hook had been in that condition and used daily for at least six months. (b) Metal Fabrication Area: on or about and at times prior to April 24, 2024, a hook used with the 15-ton Cyclops S/N 1297 had a safety latch which was damaged and did not make contact with the hook. The hook was used to move steel transformer housings weighing up to 15-tons. The hook had been in that condition and used daily for at least six months.
