29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices (RAGAGEP). On or about April 9, 2024, in Alky Unit 22 at Phillips 66 Company located in Borger, Texas; the employer did not document that equipment complies with their adopted RAGAGEP, ASME Boiler and Pressure Vessel Code, Section VIII Division 1, 1968 Edition, Appendix M, Section UA-354 (a) (Stop Valves Between Pressure Relieving Device and Vessel) to ensure that block valve on the inlet of the relief valve R-326 on the Depropanizer Condensate Keg (95- RA06) was locked or sealed in the open position.
