29 CFR 1910.23(b)(8): The employer did not ensure that ladders are used only for the purposes for which they were designed. a) On or about 04/15/2024 at the site located at 3000 Milton Ave, Solvay, NY 13209: Employees climbed a Werner Manhole ladder to insert a shovel at the bottom of the door for Kiln #10 exposing employees to a fall distance of approximately 3 feet. The ladder rungs on the Werner Manhole ladder were approximately 9.5 inches wide. b) On or about 04/15/2024 at the site located at 3000 Milton Ave, Solvay, NY 13209: Employees climbed Werner Manhole ladders to access the sand funnel for core machines #4 and #1 exposing employees to a fall distance of approximately 6 feet. The ladder rungs on the Werner Manhole ladders were approximately 9.5 inches wide.
