29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): (a) CGRS, Inc., 1525 - 1555 Sherman St. (Alley), Denver, CO 80203: The employer did not ensure effective protection against cave ins, as demonstrated on or before March 20, 2024, and times prior, when employees were exposed to cave in hazards while excavating and removing an underground fuel tank when the excavation was greater than 5 feet deep.
