29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: (a)Mario Castillo at 27142 Park Lane Drive, Tea, SD 57064: On or about and at times prior to February 5, 2024, the employer did not ensure that scaffolding training was provided to employees who were conducting work from an elevated scaffold system prior to conducting roofing operations on a commercial property. This condition exposed employees to fall hazards greater than ten feet.
