29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedures at least annually to ensure that the procedures and the requirement of this standard were being followed: a)T & J Industries, Inc dba Metal Craft Industries at 1014 South Hillside Drive, Sioux Falls, SD 57110: On or about and at times prior to December 6, 2023, the employer did not ensure a periodic inspection was conducted at least annually to ensure that the lockout/tagout program was being followed and any deviations or inadequacies were identified. This condition exposed employees to amputation, crushing and shock hazards associated with an improperly locked out equipment.
