29 CFR 1910.28(b)(15): Walking-working surfaces not otherwise addressed. Except as provided elsewhere in this section or by other subparts of 29 CFR part 1910, the employer must ensure each employee on a walking-working surface 4 feet (1.2 m) or more above a lower level is protected from falling by: a. On or about April 10, 2024, and at times prior, the employer did not ensure that employees working from the maintenance mezzanine for the Universal Sorter Machine were protected from the step-through fall hazard from the mezzanine access platform's walking-working surface for where the F-3 belt connects to the F-5 belt. As a result, employees were exposed to fall hazards of approximately 13 feet to the lower level.
