29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): On or about October 27, 2023, and at times prior to, employees were engaged in work activities related to installation of pipe in a trench greater than 4-feet deep and 25-feet long were not protected by protective systems such as, but not limited to shielding, benching, shoring, or sloping.
