29 CFR 1910.146(d)(1): Permit-required confined spaces. Permit-required confined space program (permit space program). Under the permit space program required by paragraph (c)(4) of this section, the employer shall: Implement the measures necessary to prevent unauthorized entry; a) On or about October 14, 2023, employer did not ensure that there was measures in place to prevent unauthorized entries into the Line 2 ARG-VPL Lamination Ring Puller Machine in that any employee working on Line 2 ARG-VPL Lamination - Ring Puller machine was allowed to enter through the 9" inch opening or through the double doors and conduct the necessary valve adjustment located inside of the permit required confined space area. This was last observed on October 24, 2023.
