29 CFR 1910.178(l)(3)(ii)(I): Other unique or potentially hazardous environmental conditions in the workplace that could affect safe operation. Training program content. Powered industrial truck operators shall receive initial training in the following topics, except in topics which the employer can demonstrate are not applicable to safe operation of the truck in the employer's workplace. Workplace-related topics: a.) On or about September 29, 2023, the employer did not ensure that power industrial vehicle operators and trainers received initial training for workplace related topics such as, but not limited to unique and hazardous environmental conditions caused when forklift operators were not adequately trained to load the pallets of straw onto the flatbed semi-trailer utilizing the standup forklift. And the Toyota Forklift Truck Model: 50-8FCCU25 Serial #: 14352 with the 15-16 foot double probes to first load the pallet of straw with the standup and then to turn the pallet of straws with the probe.
