29 CFR 1910.178(m)(3): The employer failed to ensure unauthorized personnel were not permitted to ride on powered industrial trucks. A safe place to ride shall be provided where riding of trucks is authorized. a. On or about September 11, 2023, the employer failed to ensure that an unauthorized employee was not permitted to ride on a powered industrial vehicle. The affected employee was exposed to tip over and crush hazards while standing on the back of the Komatsu 45 Model: FG45BCS-7 forklift while another employee operated the forklift to pull a bundle of 12 quarts slabs from a closed-top shipping container.
