Text For Citation: 01 Item/Group: 002B Hazard:
29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: On or about 8/31/2023, the employer exposed authorized employees to caught in hazards in that, written lockout/tagout procedures were not specific for machines, such as but not limited to, the V-Process automated molding machine, Model V.N.A, serial no P-06601204: (a) Mold Department at 55 Traverser: The Lock-out instruction did not address the magnitude of the energy source and the specific location of the of the disconnecting switch. (b) Mold Department at 55 Cut Heater: The Lock-out instruction did not address the thermal energy as source and the magnitude of the electrical energy source. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
