29 CFR 1910.178(l)(3)(i)(J): The employer did not provide training for any vehicle inspection the operator will be required to perform. On or about August 22, 2023,employees were performing material handling work using powered industrial trucks at a workplace located at 4140 Webster Street, Springfield, Missouri, where they were exposed to struck-by hazards. Specifically, the employer failed provide training to employees on how to inspect the powered industrial trucks for conditions such as, but not limited to: L.P. or gas leaks, oil level and pressure, brakes, lights, horn works, hour meter, steering, tires, hydraulic controls, hydraulic lines, check for leaks, overhead guard, battery plug connection and battery charge. One instance of this failure was an employee operating the Toyota forklift without a functional back-up alarm..
