29 CFR 1910.28(b)(3)(i): The employer did not ensure that each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following methods listed in 29 CFR 1910.28(b)(3)(i)(A)-(D): Covers; Guardrail systems; Travel restraint systems; or Personal fall arrest systems. a) Jobsite: 7800 Penrose Ferry Rd, Philadelphia, PA 19153 - On or about August 23, 2023, the employer did not ensure employees replacing air scrubber media on the roof of a commercial building, approximately 17 feet above the lower level, were protected from falling through skylights by the use of covers, guardrails, travel restraint systems, or personal fall protection systems.
