29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment: a) Worksite was located at 2 Applegate Drive, Robbinsville Twp, NJ 08691: Workers tasked with working on Bag Dump machines were not the workers locking the machines out nor did they have locks to use as part of a group lockout to secure these machines during the deep cleaning process. Violation occurred on or about August 20, 2023. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
