29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): (a) Coating room; On or about August 16, 2023, energy isolation devices were not used to control electric, pneumatic, and gravitational energy sources on the HST oven and related conveyor systems before an employee entered the machine enclosure to perform servicing and maintenance activities of troubleshooting the function of inlet side conveyor components. An employee's finger was partially amputated after being crushed by an exposed cylinder when it retracted to lower the inlet belt conveyor during production operations. (b) Coating room; On or about August 14, 2023, energy isolation devices were not used to control electric, pneumatic, and gravitational energy sources on the HST oven and related conveyor systems before four employees entered the machine enclosure to perform servicing and maintenance activities of manually cleaning debris from internal machine and conveyor belt surfaces. (c) Coating room; On or about August 14 through 16, 2023, energy isolation devices were not used to control electric, pneumatic, and gravitational energy sources on the HST oven and related conveyor systems before an employee entered the machine enclosure to perform servicing and maintenance activities of removing, cleaning, and replacing material stop pins on the inlet side conveyor system. (d) Coating room; On or about August 16 through August 18, 2023, energy isolation devices were not used to control electric, pneumatic, and gravitational energy sources on the HST oven and related conveyor systems before two employees entered the machine enclosure and knelt down to perform servicing and maintenance activities of tensioning bottom rack return conveyor belts.
