29 CFR 1926.1053(b)(15): Use. Ladders shall be inspected by a competent person for visible defects on a periodic basis and after any occurrence that could affect their safe use. a) On August 16, 2023, the employer did not ensure that a portable extension ladder was adequately inspected prior to use in that employees were using a deconstructed and damaged (bent rung and uprights) extension ladder as the means of access and egress for an open excavation.
