29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s). a) Warehouse Department (Facility - Forklift Operations): On or about August 08, 2023, and at times prior, the employer exposed employees to struck-by and crush-by hazards in that the employer failed to take out of service a KOMATSU Forklift Truck: Model No. FG 188T-17 with unsafe parts such as but not limited to leaking propane and broken front signal lights while employees were required to perform forklift operations.
