29 CFR 1910.147(c)(6)(ii): The employer shall certify that the periodic inspections have been performed. The certification shall identify the machine or equipment on which the energy control procedure was being utilized, the date of the inspection, the employees included in the inspection, and the person performing the inspection: Employees engaged in equipment servicing and maintenance activities at Kinney Manufacturing, LLC doing business as IntoMetal located at 3340 North 33rd Street, Lincoln, Nebraska, were exposed to caught-in hazards in that the employer failed to certify lockout/tagout periodic inspections were performed for 40 of 41 procedures.
