29 CFR 1926.417(b): Equipment or circuits that were de-energized were not rendered inoperative and did not have tags attached at all points where such equipment or circuits could be energized: On or about May 15, 2023, and at times prior to, Taxiway L (Lima), a series circuit was in operation and did not have tags when employees worked on the circuit, exposing employees to electrical hazards.
