29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: a) Production Areas (Facility - Fabrication/Finishing Department): On or about May 08, 2023, and at times prior, the employer exposed employees to amputation hazards, in that an energy control program (Lock Out/Tag Out-LOTO) training was not provided to employees performing service and maintenance activities such as but not limited to "end of day" equipment cleaning on the AccurShear (Model No.: 625010 Serial No.: 6637).
