29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On or about May 5, 2023, two maintenance technicians were engaged in maintenance and servicing activities on a MOBA MR12 Case Packer #4 line a. Isolation devices were not physically located and operated in such a manner to prevent the unexpected re-energization of the conveyor belt and robotic arm. Circuitry controls (i.e., interlocks) were used as the primary method of de-energization.
