29 CFR 1910.119(i)(2)(i): The employer did not ensure that the construction and equipment is in accordance with design specifications; On or about March 22, 2023, in Cumene Plant at INEOS Phenol located in Pasadena, TX, workers were exposed to fire and explosion hazards, when the employer did not ensure that the pipe specification#11 were followed for design, engineering and construction of four (4) LPG/propylene unloading arms (A thru D) at truck unloading stations.
