29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel. a) Worksite: On or about 3/1/2023, the Toyota forklift, Model 7FGCU32, serial #65245, had not been taken out of service where three tires showed significant damage, to include cracks, splits, and missing chunks of the tire. b) Worksite: On or about 3/1/2023, the Toyota forklift, Model 7FGCU32, serial #65245, was not repaired when the load back rest broke away from the forklift carriage.
