29 CFR 1910.119(j)(4)(iii): The frequency of inspections and tests of process equipment was not consistent with applicable manufacturers' recommendations and good engineering practices, and more frequently if determined to be necessary by prior operating experience: a) In the Engine Room, on or about January 18, 2023, the frequency of inspections of anhydrous ammonia refrigeration system process equipment was not consistent with Bulletin No. 109 Guidelines for: IIAR Minimum Safety Criteria for a Safe Ammonia Refrigeration System, Section 5.3, because a more thorough inspection of the ammonia refrigeration system was not conducted every five years. The employer selected the bulletin to use as guidelines for their in-depth inspection of critical refrigeration equipment. Failure to inspect anhydrous ammonia refrigeration system process equipment at appropriate frequency can lead to loss of containment and exposure to toxic and fire hazards.
