29 CFR 1910.147(c)(7)(i)(A): Each authorized employee had not received training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: On or about February 21, 2023, an employee was exposed to caught-in and amputation hazards when performing maintenance and servicing on the Accurpress and Beyeler Xpert Bystronic Press and the authorized employee was not trained to isolate and/or render energy sources safe prior to performing service and maintenance task.
