29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On or about November 22, 2022, laser operator did not affix energy isolating devices to the Bystronic laser material handling lift when adjusting material handling lift suction cups vacuum spigots. Sub-Zero, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(d)(4)(i), which was contained in OSHA inspection number 1449489, citation number 1, item number 2b and was affirmed as a final order on May 06, 2020, with respect to a workplace located at 6061 Basswood Drive, Madison, WI 53719.
