29 CFR 1926.502(d)(17):The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. a. On or about July 26, 2022, at the above addressed jobsite, an employee conducting commercial steel tubing installation operations was wearing a personal fall arrest system (PFAS) with the attachment point at the employee's back and also at the employee's front, thus the harness was being worn upside down, near their waistline, exposing the employee to fall hazards.
