29 CFR 1926.1204(c): The employer did not develop and implement the means, procedures, and practices necessary for safe permit space entry operations, including, but not limited to, those specified in subparagraphs �1926.1204(c)(1) through �1926.1204(c)(8): a) At the jobsite, where employees entered storm water lines, the permit program developed and implemented did not specify acceptable entry conditions; did not contain the procedures for observation of monitoring by employees; did not contain the procedures to isolate the storm water lines; did not contain the procedures for purging, inerting, flushing and ventilating the storm water lines; did not contain the procedures for emergency exit of the storm water lines if the ventilation failed; did not contain the procedures for protecting the storm water lines from pedestrians and vehicles; did not contain the procedures for continuous monitoring; and did not contain the precautions for the removal of the manhole cover, exposing employees to inhalation and engulfment hazards.
