Text For Citation: 01 Item/Group: 002B Hazard:
29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a. On or about and prior to February 22, 2022, the employer failed to apply energy control procedures for covered activities performed on Milling Machines, including the Kitamura MyCenter HX800iL (HM-21), the OKK MCH 600 (HM-22), the OKK MCV 820 (VM-11), the YAMA Seiki machining center (VM-12) and the Mitsubishi MVR 30 (FM-51). The employer did not assure that the following elements and actions were done in the required sequence, in that operators did not have knowledge in the methods or means to control milling machine hazardous energy sources, that milling machines were shutdown or turned off using compliant machine specific procedures, that milling machines were not isolated by the use of energy isolating devices, such as electrical disconnects, that lockout devices (or tagout devices) were not affixed to energy isolating devices to assure the devices are in an "off" or "safe" position and that verification of isolation is accomplished prior to the performance of work covered by this part. Machine operators performed covered activities, including tool changes and cleaning, without the performance of the actions required by 29 CFR 1910.147(d)(1)(-(6). During covered activities, machine operators have exposure to potential struck-by and caught-in hazards associated with milling machine tooling apparatus and tools.
