29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) Marine Tech Wire & Cable, 631 South Richland Avenue, York, Pennsylvania - Employee machine operators did servicing and maintenance work, such as cleaning and set-up, on machinery and equipment, to include the Wardwell 16-carrier braiders, and the employees did not lockout the machinery and equipment, exposing employees to hazards, including but not limited to, getting caught-in and struck-by moving machine parts, on or about February 9, 2022. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
