29 CFR 1910.119(e)(3)(iii): The process hazard analysis did not address engineering and administrative controls applicable to the hazards and their interrelationships such as appropriate application of detection methodologies to provide early warning of releases: a) Polymerization Plant 2: The process hazard analysis (PHA) did not address administrative controls including the use of fire-resistant clothing (FRC) on the process pad that had an electrical classifications of Class 1 Division 2. Failure to address these controls can result in inadequate protection of employees during a release and employee exposure to fire or explosion hazards, on or about February 2, 2022. b) Polymerization Plant 2: The process hazard analysis (PHA) did not address administrative controls including the use of fire-resistant clothing (FRC) in the pump room that had an electrical classification of Class 1 Division 2.
